After your first appointment has been scheduled, it is important
for you to gather as much information as possible and bring it with
you to that meeting, so that we can efficiently and effectively
help you resolve your problem.
Documents that should be collected
include:
- medical records
- accident reports
- insurance policies
or other insurance information
- estate documents (powers of
attorney, letters of guardianship, wills, or letters testamentary)
- any invoices for bills which you have been asked to pay
or which you have already paid.
Additionally, please print
out and complete the appropriate form listed below for
the type of appointment you have scheduled (highlighted fields indicate
necessary information; other fields should be completed
only if you have sufficient information to answer the question):
